What are we looking for in a Phone Outreach Facilitator?
The Phone Outreach Facilitator is part of our Home Health Care Coordination team, which provides support to individuals who have chronic health conditions and who are transitioning from inpatient stay or are high users of the healthcare system. This position utilizes phone outreach to engage clients and provide agency-wide support for the Health Home Care Coordination program. In addition, the Phone Outreach Facilitator is responsible for the administrative duties of conducting eligibility reviews, inputting client data, and completing monthly billing reconciliation. This position requires sincere compassion for clients and their health, strong interpersonal skills to build rapport with clients and staff, and exceptional phone communication skills. This position is based in our Yakima office and may provide support to the Health Homes team members from all locations.
Phone Outreach Facilitator duties may include:
contacting beneficiaries by phone to provide an introduction and overview of available care management services
assisting in scheduling introductory home visits. Coordinates with care management staff to facilitate initial and ongoing appointments
demonstrates and communicates a sincere passion for patients health. Demonstrates excellent communication and phone skills with each contact.
language used is respectful, clear, descriptive, concise and patient-centered
completes all required documentation accurately and in a timely manner
Qualifications:
High school diploma/GED
Two years of related experience
