Job Description

Position Overview:

As a Mobile Wound Care Nurse Practitioner, you will be responsible for providing expert wound care management in patients' homes and/or care facilities. You will assess, diagnose, treat, and educate patients with chronic and complex wounds, including but not limited to diabetic ulcers, pressure ulcers, surgical wounds, and other skin-related conditions. You will work collaboratively with physicians, nurses, and administrative leadership to ensure the best possible care for our patients.

Key Responsibilities:

Conduct comprehensive assessments of patients with acute and chronic wounds, including diabetic ulcers, pressure ulcers, venous ulcers, and surgical wounds.

Develop, implement, and evaluate individualized wound care treatment plans based on patient needs.

Perform wound debridement, dressings, and other advanced modality wound care procedures.

Collaborate with interdisciplinary teams, including physicians, nurses, physical therapists, and dietitians, to ensure optimal patient outcomes.

Educate patients and their families on wound care, prevention, and self-management strategies.

Monitor and document patient progress, including wound healing and response to treatment.

Prescribe medications, such as antibiotics or topical treatments, as appropriate for wound management.

Stay updated on the latest evidence-based practices and wound care technologies.

Provide leadership and mentorship to nursing staff and other healthcare providers.

Qualifications:

Master’s degree in Nursing (MSN) or Doctorate in Nursing Practice (DNP) from an accredited program.

Active Nurse Practitioner (NP)/Physician Assistant (PA) license in Oklahoma.

Wound Care Certification (WCC) or board certification in wound care (preferred).

Minimum of 2 years of clinical experience in wound care or a related field.

Strong knowledge of wound healing, wound care products, and advanced wound therapies.

Excellent interpersonal, communication, and organizational skills.

Ability to work collaboratively in a multidisciplinary team environment.

Strong critical thinking and problem-solving abilities.

Current BLS (Basic Life Support) certification; ACLS (Advanced Cardiovascular Life Support) is a plus.
