As a Patient Access Assistant at Horizon, you will…

Welcome all clinic visitors to create an exceptional patient experience with every interaction.

Work in a demanding, fast-paced environment that frequently adapts to industry standards, interacting with patients seeking mental health and substance use treatment. Patients can include children as young as three, adolescents, and adults 18 and older.

Utilize multiple electronic platforms, including our Electronic Health Record (EHR), various web-based systems, and other technology platforms. Duties include overseeing scheduled appointments, assisting with medication refill requests from patients and pharmacies, preparing charts and record requests, and other tasks as needed.

Support the day-to-day needs of our clinical and medical staff, including managing, triaging, and supporting administrative responsibilities. Use supplemental technologies to support patient and clinical care, which can include handling insurance and billing disputes, scheduling appointments, coordinating transportation needs, and communicating with referral sources from courts, schools, and hospitals.

Employ creative problem-solving to support the overall operations of the clinic.

Serve as the primary contact for new patients during open access hours, providing a welcoming and supportive environment.

Assist with the intake and registration processes, including completing necessary forms and obtaining required releases.

Address patient questions and explain the intake process, CCBHC services, and other relevant information.

Ensure all patients are signed up for the patient portal, offering guidance and support as needed.

Arrange for translation services when necessary to ensure effective communication with patients.

Identify and address any obstacles that may prevent patients from engaging in treatment, working to find solutions to enhance patient access and retention.

High school diploma with 2 years of experience within the customer service or behavioral health field required.

Healthcare experience preferred.

Bilingual proficiency in Spanish & English required to be considered for pay differential.

Location:

This position is specifically for our 3020 Bailey Ave, Buffalo NY clinic!

Position Hours:

You’ll coordinate a personalized schedule with your supervisor that will meet both the clinic and patients’ needs as well as yours. Our outpatient clinics offer expanded hours Monday – Thursday so we’ll ask you to be available at least 2 evenings per week, but don’t worry about jeopardizing your work-life balance… we can help you create a schedule that works for you, as many locations close early on Fridays!

Compensation:

Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $18.00 - $19.00. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.

*Please note: this is a grant-funded position.

Physical demands associated with this position include:

Sitting for extended periods, alternating between standing and sitting

Mobility required with repetitive wrist, hand, and finger movements

Handling light duties, occasionally lifting objects up to 20 pounds
