Community Health Worker

Community Health Worker - Hardwick Area Health Center Northern Counties Health Care is seeking a dedicated and compassionate Community Health Worker (CHW) to join our team at the Hardwick Area Health Center in beautiful Hardwick, Vermont.

This full-time (32 hours/week) position plays a vital role in supporting patient wellness and improving access to care by bridging health services and the community.

Responsibilities: * Build trusting relationships with patients to support their medical, behavioral, and social needs.

* Conduct structured assessments in various settings, including homes, offices, and the community.

* Assist patients in accessing resources like food, housing, transportation, and legal services.

* Provide supportive coaching and advocacy to help patients set and achieve health goals.

* Maintain accurate records, track outcomes, and report on progress and client satisfaction.

* Participate in community wellness programs and represent NCHC at local outreach events and meetings.

Qualifications: * High School diploma or GED required; Associate's Degree preferred.

* At least one year of experience in a healthcare or human services setting.

* Strong communication and interpersonal skills with a compassionate approach to care.

* Proficiency in computer skills, including email, spreadsheets, and documentation.

* Reliable transportation and the ability to travel locally.

* Ability to work independently and collaboratively as part of a team.
