Job Summary:

Working closely with the Thoracic Oncology Interdisciplinary Team's (IDT) Physician Leader, will oversee the patient navigation and coordination of full care of patents in the Lung Nodule Clinic and Institutional Lung Nodule Risk Reduction Program (cancer of the lungs, esophagus and mediastinum.) This position is responsible for the full coordination and continuum of care for the patient in the program to experience expedient, optimum services. Coordination of care responsibilities will include but will not be limited to ensuring complete medical documentation to guide the treatment planning; prioritizing and coordinating screenings, diagnostic tests, laboratory tests and ensuring required insurance processes are completed such as pre-certifications, pre-authorizations, medical necessity certificates (etc.). This position will serve on the treatment team and identify patients eligible for available clinical trials. The position will be responsible for preparing for and organizing case presentations at Multidisciplinary Thoracic Oncology Tumor Board. In carrying out these responsibilities, the position will coordinate with other RN's, Medical Office Assistants, and Certified Nursing Assistants that serve on the Thoracic Oncology team. This position makes clinical judgments based upon synthesis of current research, concepts, principles, theories, and outcomes to solve complex problems.

Education & Experience

Three years of RN experience working with a chronic disease, utilization review, and/or quality management. Evidence of care coordination across continuum of care is preferred.

CERTIFICATIONS, LICENSES OR REGISTRATION REQUIRED: Valid RN license

Knowledge, Skills & Abilities

Knowledge of evidence-based clinical guidelines across a wide variety of conditions and age groups. Knowledge of care management and resource/utilization management. Skilled in the use of personal computers and related software applications. Ability to manage multiple priorities under time constraints. Ability to analyze and solve problems. Understanding cost and quality issues. Verbal and written communication skills. Interpersonal skills to work and thrive as a key part of a dynamic multidisciplinary team, with strong presentation skills and ability to interact with a wide range of constituencies. Knowledge of healthcare industry trends, medical procedures, patient assessment, regulatory requirements, and medical insurance provisions and processes.

RESPONSIBILITIES

Receive all new referrals for the Thoracic Oncology IDT. Determine urgency and timing of clinic visit. Coordinate imaging and diagnostic studies. Make appropriate evaluation referrals. Coordinate with the director of the Lung Nodule Clinic to ensure lung nodules referred to the Lung Nodule Clinic by the Radiology Department Lung Nodule Scoring and Alert System (LungRads) are assessed for urgency and timing of clinic visit. Responsible for preparing and organizing case presentations for the Thoracic Oncology Tumor Board.

Coordinate care across the specialties of Medical Oncology, Pulmonary, Gastroenterology, Thoracic Surgery, Radiology, and Radiation Oncology to minimize time from presentation to treatment plan. Arrange appointments to provide "one-stop shopping" across the Cancer Institute. Ensure insurance approval and Financial Assistance compliance across phases of care. Ensure standard of care and best practices are employed across the service line. Communication with referral sources.

Ensure new referred patients that meet Lung Screening Criteria are referred to the Lung Nodule Clinic, scheduled for screening exam and follow up visit.

Collect cases. Organize Records. Document discussion in the medical record.

Provides education and preventive health measures while helping the patient with chronic disease or condition to set behavior change goals.

Participates in the control of cost effective services that reflect appropriate levels of staff, proper use of equipment, appropriate use of supplies, and effective management patient care.

Adheres to restorative care philosophy by incorporating principles into direct care provided to residents on a daily basis.

Evaluates, develops, and implements clinical program components to optimize patient care strategies and goals.

Demonstrates effective interviewing and assessment skills and integrates new or current techniques to obtain information from the resident/family to plan, implement, and evaluate patient care.

Provides timely internal/external customer service in a cooperative, professional, and respectful manner.
