**Purpose:**
Provides mental health service coordination, according to the level of need, to a population of individuals with a serious mental health diagnosis. The primary function of Service Coordination is to provide professional assessment, service planning, service coordination, referral, and re-evaluation services required for a safe and healthy community life which is manifested through stability in relationships, housing, and employment. This is done in accordance with the Agency's vision, mission, and values, and emerging mental health Recovery Principles. This position provides an essential recovery orientation.
**Responsibilities:**
· **Coordination and Advocacy:** Provides professional coordination of mental health services, often taking the lead role in advocacy and using discretion to promote mental health recovery.
· **Relationship Building:** Develops relationships with individuals, their families, and other important people in their lives.
· **Assessment and Planning:** Assesses strengths and needs collaboratively, develops assessments, and promotes service planning efforts to create comprehensive service plans.
· **Documentation:** Completes assessments, service plans, and other required documentation collaboratively with individuals.
· **Meeting Facilitation:** Convenes and facilitates interdisciplinary service planning meetings to ensure individuals' needs are met.
· **Resource Linkage:** Assists individuals and families in identifying, linking, accessing, and coordinating resources, both internal and external.
· **Cultural Competence:** Provides services considering the individual's racial, religious, gender, sexual orientation, age, and ethnic background.
· **Resource Investigation:** Investigates new resources and maintains an up-to-date catalog of community resources.
· **Advocacy:** Advocates for individuals to ensure responsiveness from various services and supports.
· **Training and Compliance:** Attends training programs to stay updated on best practices and reviews charts for compliance with regulations.
· **Record Maintenance:** Maintains regular contact during hospitalizations and transitions of care, keeping accurate records of service coordination activities.
· **On-Call Participation:** Participates in the On-Call Service as per department guidelines.
· Bachelor’s Degree in a Human Services field such as Social Work, Psychology or Counseling required.
· Other fields of study may be considered with coursework of at least 12 credits in Human Services.
· One-year related experience preferred. Prior experience working in community settings preferred.
· Safe driving record required.
**Licensure, Certifications, and Clearances:**
· Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
· Cardiopulmonary Resuscitation (CPR)
· Comprehensive Crisis Management (CCMC)
· Driver's License
· Act 31 Child Abuse Reporting with renewal
· Act 33 with renewal
· Act 34 with renewal
· Act 73 FBI Clearance with renewal
