PrimaryOne Health is currently seeking a Homeless Program Case Worker in our Columbus centers.

Essential Job Responsibilities:

Provide crisis assistance to help facilitate; securing food, shelter, clothing and assisting clients in obtaining a primary care medical home.

Assist clients completing initial entry forms and maintains client records.

Coordiante appointments, transportation and other social or personal services as needed.

Responsible for client follow-up after broken appointments and/or incomplete service delivery.

Successful candidates will have a high school diploma or equivalent; some college preferred in behavioral, human services, or related field.
