INTRODUCTION
Unity Health Care is seeking a skilled Chronic Care and Off-Site Clinics Database Manager to oversee the development and use of health care databases within our organization. The Manager’s responsibilities include managing and maintaining databases while ensuring high levels of data availability for medical staff. The candidate will also be responsible for developing, testing, improving, and maintaining new and existing databases to help users retrieve data effectively. As our ideal candidate, you have proven experience as a Chronic Care and Off-Site Clinics Database Manager, and you are well-versed in data administration and management process.
MAJOR DUTIES/ESSENTIAL FUNCTIONS
Off-site mention
Management of CCC enrollment and schedule
Scheduling referrals to CCC from enrolments at intake /referrals
Appointments monitoring for offsite consultations and treatments
Creation of schedules in EMR as well as printed Schedules to Medical records, officers, and providers for chronic care clinic sessions.
Scheduling in EMR- includes movement of schedules, creation of appointments and cancellation of appointments.
Assuring patients are registered in all Chronic Care correctly to match problem list.
Weekly reports consist of Weekly Data Sheet and Weekly schedules with clarification of missed appointments, transfers, releases, rescheduled refusals, offsite appointment follow ups. Reports are emailed to Data Collector, RMD, Med. Director, HSA, DON, ADON
MINIMUM QUALIFICATIONS
Bachelor’s degree in a scientific or healthcare-related field.
3-5 years of experience in clinical data management or a similar role in the pharmaceutical or clinical research industry. Strong understanding of clinical trial processes, data management principles and regulatory guidelines.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
Proficiency in database management systems (DBMS) like SQL to effectively query, manipulate, and analyze data.
Proficiency in data analysis tools like SAS, SQL, or Oracle Clinical
Expertise in clinical data management software and EDC systems
Ability to interpret complex data sets, generate reports, and identify actionable insights.
Understanding of chronic disease management, medical terminology, and healthcare regulations.
Familiarity with healthcare IT systems, including EHRs, remote patient monitoring platforms, and data integration tools.
Knowledge of data cleaning and validation techniquesSUPERVISORY CONTROLS
This position reports to the Medical Director.
