The Department of Human Services (DHS) Aging and Disability Services Division (ADSD) is hiring a Human Services Clinician II to manage the intake, assessment and referral processes for the Arlington Adult Day Program (AADP) and provide robust and comprehensive service for caregiver support across ADSD. The Clinician will manage complex clinical and psychosocial issues for Adult Day participants while maintaining communication with stakeholders regarding care coordination by utilizing social work interventions, counseling and case management services. This position will help to support the holistic needs of the participants and their caregivers as well as support caregivers with the complexities of care planning. AADP is located at the Walter Reed Community Center where personalized care is provided during the day to adults with healthcare needs and disabilities, giving them the ability to live at home longer while providing relief and support for their caregivers. Responsibilities
Specific responsibilities include:
Coordinating Medicaid services through the CCC Plus Waiver Program;
Providing ongoing caregiver support, counseling, care management and coordination to participant families;
Working with staff in exploring alternative interventions and approaches when identifying concerns with participants;
Collaborating with healthcare providers including long term care partners and faith communities to provide outreach information and education about AADP and other caregivers supports and services;
Organizing and facilitating caregiver support groups by utilizing evidenced based caregiver assessment tools;
Designing and developing initiatives to support family/caregivers in work, home, and community settings while ensuring programs are culturally competent and meet the needs of diverse groups;
Facilitating and providing training, education, and outreach to clients, gap-filling services, and caregiver counseling;
Representing the program and division at local and regional Caregiver Conferences; and
Providing a robust and comprehensive service for caregiver support across Aging and Disability Services. In addition to providing traditional caregiver supports, the position will link older adults, persons with disabilities and their caregivers to information outreach and resources in the community through options counseling, care coordination and referrals to a wide range of long-term services and supports that foster aging in the least restrictive community.

Qualifications
Minimums:
Master's Degree in Human Services, Social Work, Counseling, Gerontology, Psychology, or other directly related clinical discipline;
Practicum prior to graduation involving diagnostic, treatment, and case management services with a focus on older adults, gerontology, adult services or a related field; and
Two years of post-Master's degree experience providing professional clinical human services, which included the application of diagnostic, treatment, and case management services with an older adult population. Substitution: A directly related higher level clinical degree may substitute for the Master's degree education requirement and one year of professional clinical social work experience. In the case of such substitution, the remainder of the qualifying work experience must have been gained after the higher-level degree was awarded.
Desirables: Preference may be given to candidates with one or more of the following experiences/licensure:
Assessing clients' eligibility for public benefit programs;
Providing case management services to persons that require behavioral health or mental health counseling, with disabilities, older adults, and their families;
Managing federally mandated services;
Working with PeerPlace;
Working with individuals from diverse cultural/ethnic backgrounds; and/or
Virginia licensed or license eligible as a clinical social worker.
Applicant must possess or obtain by the time of appointment, a valid motor vehicle operator's license from the applicant's place of residence or the applicant must have the ability and willingness to use alternative methods of transportation to perform assigned duties and responsibilities at locations other than the primary worksite. The applicant must authorize Arlington County to obtain, or the applicant must provide, a copy of the applicant's official state/district driving record. Any offer of employment may be contingent upon a favorable review of the applicant's driving record.
A background check is required and may include checks of the following: criminal record, driving record, education, professional licensure, and credit check and may require signing a release authorizing the County to obtain this information. The position will require travel around Arlington County. Applicant must possess or obtain by the time the appointment a valid Driver's license from the applicant's place of resident or be willing to use alternative modes of transportation.
A pre-hire background check and State of Virginia Child Welfare and Neglect Central Registry will be required.
ADDITIONAL INFORMATION
Work Hours: Monday through Friday, 40 hours per week, between 8:00 a.m. to 5:00 p.m. Some evening and weekend work may be required. Some telework may be available, contingent upon supervisor approval and workload.
