Job Description

Responsibilities:

Educate individuals and communities on various health topics and resources

Conduct outreach activities to promote health programs and services

Assist individuals in navigating the healthcare system and accessing appropriate resources

Provide social support and advocacy for individuals facing health challenges

Collaborate with healthcare professionals to coordinate care for clients

Conduct home visits to assess needs and provide health education

Use motivational interviewing techniques to encourage behavior change

Collect data and maintain accurate records of client interactions

Skills:

Experience doing outreach and enrolling patients into health plan program

Knowledge of medical terminology and healthcare systems

Ability to effectively communicate with individuals from diverse backgrounds

Proficient in conducting public speaking engagements and facilitating group discussions

Familiarity with addiction counseling and resources

Understanding of Medicare guidelines and eligibility requirements

Experience in data collection and documentation

Pay: $25 - $30 per hour

Expected hours: Full time M-F 8-3pm (30 Hours per week)

Qualifications:

Community Health Worker Certification (Not required but preferred)

Bachelor's Degree in Social Work, Psychology, or Sociology and 2 years of experience Community Health Space

**Please note either a Community Health Worker Certification OR Bachelors Degree is required for this position**

Work Location: In person/Remote (Hybrid) in Riverside County
