Blue Cross Blue Shield and GreenWell Health have partnered to bring care to those who really need it in the state of Nevada. This is a fun and unique opportunity to be a Hybrid, Mobile Provider with a VERY reputable company. We are seeking a dedicated and compassionate Nurse Practitioner or Physician Assistant, to join our team. The ideal candidate will provide comprehensive medical care to patients, focusing on preventive care, diagnosis, and treatment of various health conditions. As a collective, our very experienced team of Mobile Providers, have over 46 years of experience working with homebound patients. This is the third successful Home-Based Primary Care Practice that our team has built.
As a mobile provider to these communities and private homes, you will assess, diagnose and treat certain conditions. It's a much needed service that is very rewarding, as these patients are very grateful. You will be helping to care for their Diabetes, COPD, HTN and other systemic conditions. Our patients reside in Senior Living Communities, Group Homes & Private Homes. This role requires a strong commitment to patient-centered care, autonomy in the work environment, as well as the ability to work collaboratively within a multidisciplinary team.
Lots of independence in this role, so it's must that you be experienced as well as self-directed. This position will be what you make it! Our providers LOVE what they do. They get to know their patients very well which makes it easy to shape the schedule in a way that makes sense for your life. When caring for these patients you can decide if you'd like to work 3 or 4 days per week. This role allows for longer days in the field, but less overall days during the week, which is highly sought after for providers with outside commitments. Once the provider knows their caseload well, a mixed approach can be taken when seeing patients, whereby the provider sees patients both In-person as well as via Telehealth visits (dependent on patient Telehealth capabilities). The hybrid, flexible schedule coupled with the mixed approach of seeing patients both in-person as well as using Telehealth, makes the role very desirable. Once you get to know everyone on your caseload, you can set things up in a way that makes sense for your life.
It's a great opportunity for a provider who's worked with seniors prior or has experience in a non-traditional work setting such as Home Health, Hospice or has previously worked in House Call Medicine. Duties
Conduct thorough physical examinations
Develop and implement individualized treatment plans based on patient needs.
Manage chronic illnesses and provide ongoing primary care for patients.
Maintain accurate medical documentation in compliance with healthcare regulations.
Provide education and support to patients regarding their health conditions and preventive measures.
Collaborate with specialists and other healthcare providers to ensure comprehensive patient care.
Address behavioral health issues as needed, providing referrals when appropriate.
Participate in memory care initiatives for patients requiring specialized attention.
Stay current with advancements in medical practices and participate in continuous professional development.
Qualifications
Nurse Practitioner (APRN) or Physician Assistant (PA-C) degree from an accredited institution.
Valid APRN or PA-C license to practice in the state.
Strong knowledge of anatomy and physiology, with the ability to apply this knowledge in clinical settings.
Basic life support certification is required; advanced certifications are a plus.
Familiarity with medical office procedures and protocols.
Excellent communication skills, both verbal and written, with strong interpersonal abilities to connect with patients from diverse backgrounds.
Commitment to providing high-quality patient care with a focus on empathy and understanding.
