Position Summary: The RN Case Manager (RNCM) is someone whose dedicated clinical expertise results in terminally ill patients receiving exceptional end-of-life care. In this role, the RNCM collaboratively works alongside an interdisciplinary team to provide end-of-life care to those needing hospice services in their home or community setting. The RNCM provides individualized case management, admission facilitation, clinical support, continuous care, after hours call, and/or other skilled nursing expertise with chronic and end-of-life care.
Salary: $83,000-92,000 annually
Job Requirements:
Succinctly and concisely convey patient assessments with clinical data to hospice physician who will then determine hospice eligibility
Provide direct patient care to terminally ill patients; assess, identify and address their individual physical, psychosocial, safety, and comfort needs
Anticipate, prevent and treat undesirable symptoms or secondary symptoms
Develop, maintain, and manage an individualized care plan that captures the evolving needs of the terminally ill patient
Train, instruct and support the patient, family and caregiver
Document patient problems, nursing assessments, patient goals, care provided and patient/family outcomes from interventions and care provided in a timely manner
Consult and collaborate respectfully with the interdisciplinary team and others involved in the patient’s care
Prepare and present all needed information for bi-weekly IDG meetings
Qualifications:
Graduate of accredited school of nursing
Current state of Minnesota RN licensure
Three years recent nursing experience in acute, long-term care or home care/hospice/public health
Patient care planning and patient teaching skills
Written and verbal communication skills
Critical thinker
Current Minnesota driver’s license, insurance and daily access to reliable vehicle
