Job Description
In this role, you will play a vital part in facilitating patient discharge planning and ensuring effective communication between specialists, clinicians, and patients' families.
Specific Duties:
This position is responsible for ensuring the continuity of care in both the inpatient and outpatient setting utilizing the appropriate resources within the parameters of established contracts and patients’ health plan benefits
Performs care management functions on-site or telephonically as the need arises and Works in conjunction with the care team and PCP as care team leader
Meets with patients, patients’ family and caregivers as needed to discuss care and treatment plan as delineated in Transitions of Care program
Identifies and assists with the follow-up of high-risk patients in acute care settings, skilled nursing facilities, custodial and ambulatory settings
Consults with physician and other team members to ensure that care plan is successfully implemented and coordinates treatment plans with the care team and triages interventions appropriate to the skill set of the team members
Qualifications and Experience:
Experience in an acute clinical care setting or equivalent and Care management, utilization review or discharge planning.
1+ years of experience in an acute clinical care setting
HMO experience
Current and valid LPN Florida state license
IV Certification
Schedule:
Shift: 8:00 AM - 5:00 PM
Days: Monday - Friday
Dress Code:
Scrub top and bottom with clean closed-toe shoes.
Scrub colors: Navy blue or black.

Additional Information:
· Location: Kissimmee, FL
· Schedule: Monday-Friday 8AM- 5PM.
· Pay Range: $30 - $31 per hour
*Offered pay rate will be based on education, experience, and healthcare credentials.
