Job Description
The Community Health Worker CHW Case Manager is a primary care clinic care team member supporting patient access to healthcare resources including substance use disorder treatment, detox, and other supportive community services. This position requires a high level of organization, teamwork, and timely responsiveness.
Job Responsibilities:
Manage a time-sensitive caseload of patients needing additional support
Obtain all required releases of information (ROI’s)
Complete referrals and necessary steps to engagement, including scheduling/coordinating assessments
Follow through and monitor referrals to support timely care
Work in collaboration with the Oasis Care Team to increase patients’ access to services
Provide patient transportation within community to support positive health outcomes
Document in electronic health record (Epic)
Strict adherence to HIPAA and privacy practices
Requirements:
High School diploma, or equivalent
Current and valid Oregon Driver’s License and able to be insured on company auto policy
Experience working with families and/or individuals affected by substance use disorders.
Community Health Worker (CHW) certification and registered in Oregon THW registry, or willingness to obtain after hire.
