Monday - Friday, 9:00AM - 5:00PM

SUMMARY

Family Service League is seeking a full-time Case Manager for the Assertive Community Treatment (ACT) program. The Case Manager conducts home visits and provides education to clients. The Case Manager also provides treatment plans and connects clients to treatment facilities.

The Case Manager will complete Entitlement Applications.

Accompany clients to Entitlement Offices.

Provide money management and budgeting skills.

Ensure active entitlements, including Medicaid and/or other benefits.

The Case Manager will provide psycho-education to families and clients.

Facilitate groups.

Complete client assessments using relapse prevention plans.

Write treatment plans.

Provide motivational interviewing.

The Case Manager will connect clients to treatment facilities.

Correspond with managed care companies.

Apply for benefits for clients (i.e., Medicaid, SSI/SSD, HEAP, Food Stamps, etc.).

Provide crisis intervention.

Cover the crisis phone.

Participate in daily team meetings and case conferences.

Conduct daily home visits.

All other duties as assigned

QUALIFICATIONS

High school diploma or equivalent required. Bachelor's degree preferred.

At least one year of experience with the severe and persistent mental illness (SPMI) population and familiarity with community resources, including DSS, SSI, Medicaid Managed Care and Housing required.

Excellent organizational and interpersonal skills required.

Strong communication skills, both verbal and written required.

Ability to multi-task and prioritize in a constantly changing, fast paced environment.

Proficient computer skills, including Microsoft Office required.

Valid and clean New York State Driver's License required.

PHYSICAL REQUIREMENTS

This position requires sitting for long periods of time and travelling to conduct home visits. Must be able to lift up to 10 lbs.
