Shift: Shift 1 Position Summary
Certified Diabetics Educator is responsible for the planning, implementing, and evaluation of diabetes education and MNT services to patients in the Middlesex Health Endocrinology clinic.. The CDE will assess the needs of the endocrine patients and identify goals in accordance with the educational standards of the American Diabetes Association. The diabetes educator will work closely with the Endocrine Providers to educate and monitor the patients and family to ascertain whether self management services provided are adequate and appropriate.
Minimum Qualifications
Current active unrestricted license to practice in the State of Connecticut as a registered nurse or registered dietitian. NCBDE certification or eligibility for certification as a CDE is required.
Self-motivated.
