Description
The Central Connecticut Coast Y's Alpha Community Services branch is seeking a compassionate and driven Case Manager to support our Family Stability Program. This role supports individuals and families experiencing homelessness or housing insecurity, providing case management services that promote long-term stability and self-sufficiency. As a Case Manager, you will work collaboratively with clients, community partners, and internal teams to connect individuals with essential resources, develop personalized support plans, and advocate for the well-being of those we serve. This position is a full-time, non-exempt role, primarily working Monday through Friday, with occasional Saturdays as needed.
Requirements
Bachelor's Degree in Social Work, Human Services, or a related field (Master's Degree preferred)
Experience in case management, with a focus on homelessness or vulnerable populations
Proficiency in motivational interviewing and person-centered planning techniques
Strong communication and computer skills
Valid driver's license and reliable transportation
Bi-lingual in Spanish preferred
Must complete YMCA-mandated training programs, including Child Abuse Prevention and Homeless Management Information System (HMIS) certification
