Are you an empathetic, organized professional who thrives in a collaborative and meaningful environment? We’re looking for a Client Support Coordinator to be a vital part of our care team—helping clients transition into treatment, supporting our clinical staff, and serving as a warm, consistent presence throughout the client journey.

This position will work with our Transitional Housing program (PHP). This is more than a job—it’s a chance to truly impact lives, every single day.What You’ll Do

Serve as a compassionate point of contact for clients and families—offering emotional support, answering questions, and helping them feel seen and heard

Manage client intakes, support transitions and discharge plans, and coordinate follow-up care

Maintain accurate, timely documentation in our electronic health records system (BestNotes)

Collaborate with clinicians and leadership to ensure smooth program operations

Organize key documents, support audits, and uphold HIPAA and licensing compliance

Provide education and resource navigation for clients and their loved ones

Keep our systems running smoothly with general administrative support and data entry

Contribute to a warm, team-oriented culture where every voice mattersWhat We’re Looking For

Bachelor’s degree in Human Services or related field

Ability to pass a criminal background check, DBHDS check, and Central Registry check

Valid driver’s license, 4+ years of driving experience, fewer than 3 moving violations in the last 3 years, and no more than 1 major violation in that time.

CPR/First Aid Certification and TB clearance required

Strong communication skills—both written and verbal

Professional, compassionate, and organized with a knack for multitasking

Proficient in Microsoft Office, Google Workspace, and Adobe

Familiarity with clinical/medical terminology a plus
