Role Overview:

Schedule: Monday to Friday (4 day work week)

Provide patient care in a skilled nursing facility by conducting rounds in their short-term units

Conduct documentation at home with provided iPad laptop within 48 hours of seeing patients

Key Responsibilities:

Perform assessments and therapy sessions

Collaborate with facility staff to enhance patient care

Ensure timely and accurate documentation

Requirements:

Hold a valid full license as either a licensed professional counselor (LPC) or a licensed mental health counselor (LMHC)

Experience in healthcare or long-term care is preferred but not required

Compensation:

Expected compensation is estimated salary range of $56,000 to $68,000 with no cap productivity income potential.
