Transitional RN Case Manager

APN Staffing is currently hiring for a Transitional RN Case Manager for our client. This position is Full Time (Monday through Friday 8:00 am to 5:00 pm). Contract position is long-term with an hourly rate of pay at $55.00 per hour.

Benefits included after 90 days on assignment

General Job Summary:

Determine care needs to include post-discharge medical and referrals for patients

Oversee the care of a patient as they move from one healthcare facility back to their home

Coordinate hospital follow up appointment for the patient

Manage post-acute care of patients that are at risk for health outcomes

General Job Requirements:

Arizona Registered Nurse license required

Bachelor’s Degree in Nursing from an accredited college or university and certification as a Registered Nurse (RN) required

Minimum of 1 – 2 years experience including Inpatient Care and/or Inpatient Case Management highly desired

Two years of clinical nursing experience in acute care or outpatient setting highly desiredCompany Description
