I. Job Summary
To evaluate and treat referred patients in order to help improve cognitive function, speech intelligibility and understanding and swallowing function.
II. Duties And Responsibilities
Performs speech language pathology evaluations and provides appropriate care using best practices.
Thoroughly documents each and every patient contact
Is compliant with all federal, state, local and insurance laws, rules and regulations regarding the provision of speech language
Communication Requirements
Checks in daily at assigned ti.mes with appropriate scheduler to review that day's schedule and inform scheduler of next day's planned schedule. Scheduler will move any missed visits that may be captured later
Communicates with scheduler immediately with any urgent scheduling needs
Contacts new patients the night before the proposed visit and offers very rough estimated visit time
Communicates SOC and Eval information to the appropriate nursing supervisor and scheduler immediately after completing visit
Contacts established patients at agreed upon times
Contacts patients immediately with any schedule changes that impact their visit
Contacts physician immediately after all evaluations, for requests for more therapy visits and for any clinically significant concerns and documents every contact in the clinical record
Responds to phone calls or text messages within 1-2 hours during work hours or immediately the next business day if messages received after work hours
Responds to emails promptly
Communicates frequently with Speech language pathology Assistants and other disciplines about patient care and coordination of care

IV. Qualifications And Experience
Graduate of an accredited speech language pathology program. Currently licensed in the state of Georgia.
Strong interpersonal skills in person, on the telephone and through electronic communication sources
Strong problem solving skills
Must have a valid driver's license
Must have own mode of reliable transportation and maintain liability insurance coverage
Must be self-motivated, a quick learner and adaptable to quickly changing situations
Must be highly organized
Must be physically able to perform the duties required of a speech language pathologist
Ability to impart knowledge to patients, caregivers and other clinicians
Must be willing to provide care outside of assigned area to ensure coverage for vacations, times of high census, meet productivity requirements,
