Our team is growing and we are searching for dedicated and energetic Full-time Service Coordinator, who will work with adults in an inpatient substance abuse treatment program. The ideal candidate would be responsible for providing direct supervisor and maintaining daily operations of the program and facility.

As a Service Coordinator, you will

Provide monitoring and assistance for each person served.

Ensures medical needs of members are met through observation of medication and proper documentation. This includes attending medical appointments and following all policies and procedures relating to med administration policies of the agency. Assures doctors' orders are followed as well.

Ensures that each member has the opportunity to engage in sober community activities at the discretion of the Clinical Supervisor.

Oversee the daily events, meals, bedtimes of clients and their children.

Facilitate Life Skills Group as indicated on the treatment schedule.

Maintain all court reports, coordinate schedules of court attendance of clients and assure timeliness of court report.

Act as Liaison between court, referral source and mental health providers.

Assure new admits have substance abuse, mental health screenings as directed by funding source and credentialing.

Conduct monthly staffing's for Oak Meadow Residential Technician and assist with developing quarterly Department Meetings. Weekly consultation with staff face to face and monthly with part time staff.

On Call Phone Duties

Requirements:

Bachelor Degree with minimum of one (1) years of full-time experience in Substance Abuse or related filed or an Associated Degree with a minimum of two (2) years of full-time experience with be considered.

Minimum of one (1) year of supervisory experience

Knowledge of co-occurring disorders

Knowledge of IDPH requirements for operating a residential substance abuse facility.

Must have a valid driver's license, a good driving record and show proof of auto insurance.

Satisfactory results on background check.
