Location: Starkville Manor Health Care and Rehabilitation

Job Type: Full-Time, Part-Time, PRN

Major Responsibilities

Provide direct patient care to residents and patients of the health and rehabilitation center (skilled nursing home).

Conduct and document a thorough evaluation of each resident's medical status upon admission and throughout the resident's course of treatment.

Assist in the implementation of individualized treatment plans for each assigned resident.

Oversee Falls Program and Pressure Sore Program.

Supervises staff and oversees patient care in accordance with standards of care and practice and applicable state/federal regulations.

Follows established facility policies and procedures while providing and coordinating resident care with other facility personnel.

Supervises Certified Nursing Assistants during shifts and participate in their evaluation.

Minimum Qualifications

Must be a Licensed Practical Nurse (LPN) with an active, unencumbered state license.

Previous experience in long-term care, skilled nursing, or geriatric nursing preferred.

Strong clinical skills, excellent communication, and a passion for patient-centered care.

Ability to multitask, work in a fast-paced environment, and adapt to changing resident needs.
