About the Role:

We are seeking a highly motivated and compassionate Community Health Worker to join our team as a Diabetes Peer Support specialist. In this role, you will be responsible for providing support and education to individuals living with diabetes in the community. You will work closely with healthcare providers to ensure that patients receive the necessary care and resources to manage their condition effectively. Your efforts will contribute to improving the overall health outcomes of the community.

Minimum Qualifications:

High school diploma or equivalent

Completion of a Community Health Worker training program

Experience working with individuals from diverse backgrounds

Strong communication and interpersonal skills

Ability to work independently and as part of a team

Preferred Qualifications:

Certification as a Community Health Worker

Experience working with individuals living with diabetes

Bilingual in English and Spanish

Responsibilities:

Provide one-on-one support to clients with Diabetes Type 2 participating in the Produce4Health Program (P4H)

Provide individualized anticipatory guidance, support to participate on the nutrition education classes and with the provision and usage of a produce incentive card that clients will use to buy and prepare fresh produce

Establish a network to ensure that patients receive appropriate care and resources

Maintain accurate and up-to-date records of patient interactions and progress

Participate in nutrition and health education classes for better informed-care

Skills:

As a Community Health Worker, you will utilize your strong communication and interpersonal skills to provide education and support to individuals living with diabetes. You will also use your ability to work independently and as part of a team to collaborate with health providers and community organizations. Your knowledge of diabetes management and prevention, as well as your experience working with diverse populations, will be essential in providing effective support to our patients. Additionally, your fluency in English and Spanish, if applicable, will be highly valued in our community outreach efforts.
