Hiring: MDS Coordinator

Location: Huntington Beach, CA

Schedule: Full-time Max 1-hour commute

What You'll Do

️ Lead MDS Assessments: Ensure accurate & compliant data collection for residents

️ Develop & Implement Care Plans: Collaborate with interdisciplinary teams

️ Participate in Quality Assurance: Identify areas for improvement & corrective actions

️ Educate & Train Staff: Provide guidance on MDS processes & best practices

️ Stay Updated on Regulations: Maintain compliance with long-term care guidelines

What You Bring

RN or LPN License (Active & Unrestricted)

Minimum 2 Years of Long-Term Care Experience

Proficiency in MDS 3.0, PDPM & Electronic Health Records (EHR)

Resident Assessment Coordinator–Certified (RAC-CT) (Highly Preferred)

Exceptional Communication & Leadership Skills
