The Physical Therapist will screen, evaluate, re-evaluate, discharge plan, and make revisions in any established plan of care as supervised by the physician.

Requirements include a Bachelor's, Master's, or Doctorate in physical therapy, certification and licensure, active state license, organizational and time management skills, excellent communication skills, knowledge of Functional Pathways policies and procedures, ability to supervise Physical Therapy Assistants, and completion of all required screenings, certifications, and licensure.
