Where you fit in
The Health Coach will provide expert consultation and coordination of wellness services to customers, provide successful strategies and programming to facilitate the integration of Health Promotion Services to customer companies and will develop and maintain relationships with customers that are consistent with the Health Promotion Services Strategic Plan.
Your exceptional qualifications
Education
Bachelor’s Degree in Health Promotion, Exercise Science, Physical Education, Nursing or Health Related Degree (Required)
Experience
Two years of experience preferred in health related field.
Your exceptional skills
Provide health coaching services to Community Health Network employees and/or corporate clients.
Incorporates appropriate behavior change strategies into the delivery of health promotion events.
Assists in planning, developing, and implementing health promotion programs based on the customer’s needs.
Ability to design, develop and present workshops, presentations and training programs to internal and external customers, including but not limited to, smoking cessation, self care, etc, based on customer needs.
Self-initiates and creates opportunities to further develop the vision throughout the continuum of care.
Works with multi-disciplinary team in planning, facilitating, and coordinating health promotion events, classes and activities.
Continually evaluates effectiveness of health promotion events and makes appropriate improvement based on customer feedback and program outcomes.
Makes appropriate customer referrals to the sales team for additional corporate health services.
Basic computer skills.
Ability to collect and record appropriate biometric data.
Ability to travel between locations
Ability to facilitate group processes, such as smoking cessation
Ability to perform and interpret the following screening activities, but not limited to\: blood pressure, body mass index, cholesterol
(HDL, LDL, and triglycerides), and glucose
Ability to plan and organize education and intervention programs
Current knowledge of health promotion and prevention trends
Excellent customer service and project coordination skills
Knowledge of and ability to apply multi-disciplinary approaches across the continuum of Health Care Services
Knowledgeable of health promotion and healthy lifestyle choices
Must have own reliable transportation to perform onsite client work
Ability to communicate effectively both orally and in writing
