RN - Hospice Liaison- Rockland County, NY

Job Type - Full-Time (Onsite)

Shift - Standard Schedule

Job Summary:

Your responsibilities include developing a hospice presence in the hospital, attending and facilitating rounds and care conferences, including, conferring with all care healthcare providers with the primary purpose of increasing UH admissions, and performing hospice admissions as needed. Our employees must have an understanding and commitment to the hospice philosophy of care. Collaborating with a diverse patient population, you must be able to communicate effectively, while sustaining a caring, kind, and professional disposition. You will be expected to maintain the highest level of confidentiality concerning patient and family information.

Qualifications/Requirements:

An associate's degree is required, and a bachelor's preferred.

Active NY state license as a Registered Nurse.

Prior RN experience is required, experience in hospice is strongly preferred.

Being bilingual in Spanish is plus, not required.

A valid NY or NJ State Driver's License, the ability to drive is a must.
