Full-time Opportunity: 38-40 hours a week.

Job Summary:

Responsible for following the Person Center Service Plan (PCSP), documenting program

results and communicating with their immediate supervisor about any progress, concerns,

etc. Must be able to work a flexible schedule and fill in hours as needed. This position entails

a high degree of accountability and consequence of error. Ability to stay in close and timely

communication with others is crucial.

Essential Job Functions:

-Assisting clients with daily needs, such as: Grocery shopping, Running errands, participating in community activities, Personal hygiene and cares, med administration, two person transfers, the use of a hoyer.

-Working with multiple individuals a day assisting with goals like: cooking, cleaning, community integration, and coping skills.

-Take clients to appointments as necessary.

Requirements: High School Diploma or equivalent. 1 year of DSP or CNA experience is not required, but preferred. Thorough training will be provided during your employment to excel at this position! Must have a valid driver's license, a good driving record and show proof of auto insurance.
