Position Purpose

Responsible for providing oversight and ensuring quality services for individuals with intellectual and developmental disabilities in accordance with the Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD) policies and regulations. The DDP will serve as a key liaison between support staff, residents, families, and regulatory agencies, and ensure that individual service plans (ISPs) are implemented appropriately, progress is monitored, and documentation is completed timely and accurately.

Required Qualifications

Bachelor’s degree in human services field (including but not limited to: social work, psychology, sociology, nursing, counseling, or related discipline) from an accredited college or university.

At least two years of experience in the field of developmental disabilities.

Must be at least 21 years of age.

Valid Georgia Driver’s License, with acceptable motor vehicle record and proof of insurance per company policy.

Must meet all requirements under DBHDD policy including successful completion of criminal background checks, drug screening, and tuberculosis screening.

Major Duties and Responsibilities

Develop, monitor, and revise Individual Service Plans (ISPs) as required, ensuring goals and outcomes are appropriate and reflective of individual needs.

Supervise and support implementation of ISPs and service delivery by residential staff.

Conduct assessments and ensure appropriate documentation for each individual served.

Monitor and evaluate service quality and compliance with DBHDD regulations and Annandale policies.

Train and support staff on best practices related to person-centered planning and documentation standards.

Participate in interdisciplinary team meetings and ensure active communication with stakeholders.

Conduct periodic reviews of service delivery and ensure that documentation meets funding and licensing requirements.

Assist with the coordination of medical and behavioral supports as needed.

Maintain all required certifications including Proxy Caregiver certification.

Complete all required reports and documentation in a timely and professional manner.

Serve as liaison with DBHDD, families, and other stakeholders as appropriate.

Support the mission and values of Annandale Village in all interactions and service provisions.

Participate in audits, inspections, and continuous quality improvement activities.

Provide emergency response and support during crisis situations as needed.

Personal Skills and Traits Desired/ Physical Requirements

Ability to read, write, and speak English.

Ability to problem solve and deal with a variety of concrete variables in situations where only limited standardization exists.

Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

While performing the duties of this job, the employee is regularly required to use hands; use fingers; handle or feel; reach with hands and arms; talk; and hear.

The employee frequently is required to sit, stand, and walk; climb or balance; stoop, kneel, crouch, bend, or crawl.

Employee is required to lift up to 25 pounds and/or move up to 75 pounds (with assistance).

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

Employee is required to transport (drive) and assist Villagers in and out of vehicles.

Employee is required to perform CPR/First Aid.
