Job Title: Registered Nurse/Licensed Practical Nurse

Salary: $35,880

Position 1: 30 hours

Wednesday: 1pm to 8pm

Thursday: 2pm to 8pm

Saturday: 10am to 7pm

Summary:

Ensures the health and safety of individuals by assessing individuals for specific nursing needs.

Provides prescribed medical treatment and personal care to individuals in home settings, performing the following duties. Complies with all principles of Human Rights, D.P.P.C., Department of Developmental Services, the Division of Medical Assistance and all other applicable funding regulations as adhered to by Lifeworks

Essential Duties and Responsibilities: ( include the following, but other duties may be assigned)

Provides preventative health maintenance (blood pressure check, monitoring weight, health and education) and treat any daily health care problems (including first aid and emergency problems).

Provide educational training, counseling and physical assistance when necessary to achieve ISP goals, and provides regular in-service training.

Act as an agency wide resource person to program staff, individuals, families, and care providers regarding health issues and concerns.

Maintain individual records related to medical and health progress, difficulties, and changes.

Audits medications agency wide.

Administers prescribed medications and treatments in accordance with approved nursing techniques and MAP regulations.

Observes patient, records significant conditions and reactions, and notifies Director or Physician of patient’s condition and reaction to drugs, treatments, and significant incidents.

Takes temperature, pulse, blood pressure and other vital signs to detect deviations from normal and assess condition of patient.

Attends meetings, in-services, significant medical appointments, and case conferences as necessary.

Other duties and/or assignments to work/program locations based on the needs of the company.

Qualifications:

Education and/or Experience:

Associate degree (A.A.) or equivalent from two-year college or technical school.

Language Skills:

Ability to read, analyzes, and interprets general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions form groups of managers, families and/or guardians of Individuals served.

Mathematical Skills:

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

Reasoning Ability:

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Vehicle/Travel:

Requires a legally insured and inspected automobile for use to be presented at work and available for use during working hours. If the program has an agency vehicle, staff are expected to drive it as needed.

Certificates, Licenses, Registration:

Valid and current driver’s license from the state of residency, RN registration.
