Area of Coverage: San Fernando Valley/ Northridge

**Job Summary:**

The Per Diem Registered Nurse provides compassionate end-of-life care and support to patients and their families in various settings, including private residences, hospice facilities, and nursing homes. This position offers flexibility in scheduling, allowing RNs to work on an as-needed basis.

**Responsibilities:**

1. **Patient Assessment and Care:** Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs. Develop and implement individualized care plans in collaboration with the interdisciplinary hospice team. Provide skilled nursing care, including symptom management and pain control, to promote comfort and quality of life.

2. **Medication Management:** Administer medications as prescribed by the attending physician, ensuring accurate documentation and compliance with regulatory standards. Educate patients and caregivers on medication administration, side effects, and precautions.

3. **Patient and Family Support:** Offer emotional support and counseling to patients and their families as they navigate the end-of-life journey. Facilitate open communication and assist with advance care planning, including discussions about goals of care, resuscitation preferences, and palliative interventions.

4. **Coordination of Care:** Collaborate with the interdisciplinary hospice team, including physicians, nurse practitioners, social workers, chaplains, and bereavement counselors, to coordinate comprehensive care and support services. Communicate effectively with team members to ensure continuity of care and adherence to care plans.

5. **Documentation and Reporting:** Maintain accurate and up-to-date documentation of patient assessments, care interventions, and communication with healthcare team members. Complete required documentation in electronic health record (EHR) systems and other hospice-specific databases in a timely manner.

**Qualifications:**

- Current licensure as a Registered Nurse (RN) in the state of practice.

- Minimum of 2 years of clinical experience, preferably in hospice, palliative care, or a related field.

- Strong assessment and critical thinking skills.

- Compassionate bedside manner and ability to provide emotional support to patients and families.

- Excellent communication and interpersonal skills.

- Proficiency in electronic health record (EHR) documentation.

- Certification in hospice and palliative care nursing (CHPN) preferred but not required.

**Working Conditions:**

- Per diem scheduling allows for flexibility in hours and shifts, depending on the needs of the hospice organization and availability of the RN.

- Work settings may include patient homes, hospice facilities, nursing homes, or other healthcare settings.

- Some evening, weekend, and holiday shifts may be required based on patient needs and staffing requirements.
