The Registered Nurse (RN) collaborates with an interdisciplinary team to provide compassionate hospice care addressing patients' physical, medical, psychosocial, emotional, and spiritual needs.
Key responsibilities include case management, comprehensive assessment, care coordination, supervision of nursing staff, documentation, patient/family education, and quality improvement.
The RN delivers individualized care, participates in team meetings, oversees infection control, and advocates for patient and family needs.
Requirements: current RN license, at least two years of recent hospice or home health experience, reliable transportation, and physical ability to support patient needs.
