Social Worker - LCPC / LCSW - Hospice
Job Description
Social Workers participate with the multidisciplinary team in the assessment, treatment and discharge planning as well as on going outpatient treatment as appropriate for Behavioral Health Services patients.
ResponsibilitiesCharts treatment processes and activities of patient care., Conducts individual, family and/or group treatment sessions., Develops knowledge of community resources and acts as a liaison to these agencies., Ensures data is entered accurately and timely into patients records/files., Keeps abreast of new developments in psychosocial sciences, social service delivery and treatment., Maintains awareness of current community resources and services., Offers psychosocial evaluation and short term counseling to patients and families, assisting them in their adjustment to the patient's medical condition and hospitalization., Participates with multidisciplinary team in the assessment, treatment and discharge planning as well as on going outpatient treatment as appropriate for Behavioral Health Services patients., Provides crisis intervention/support to patients and involved throughout the Health Center, as well as the Regional Behavioral Health Network as appropriate., Provides professional consultation to hospitals, Home Health Agencies, and community residential facilities, through the application of social work assessment, intervention, and treatment techniques, according to contract specifications.
