RIVER BEND TRANSITION ACADEMY

Favarh's River Bend Transition Academy is a unique, school-to-college transition program for young adults with disabilities. The Academy uses a combination of classroom instruction coupled with collegiate education to prepare individuals with developmental and intellectual disabilities for academic success and competitive employment.

Position Description:

The Academic Success Tutor reports directly to the Transition Coordinator and is responsible for implementing procedures and individual programs that will enhance opportunities for students learning at River Bend Transition Academy as part of the Favarh Transitional Service Program. Responsible for the on-going support of the Transition Coordinator in the management of daily operations, service delivery, facility, equipment, transportation, program facility cleanliness, health and safety, and overall implementation of policy and procedures. As well as actively involved in the organization and implementation of project assignments and actively participates with planning special events.

Responsibilities:

Provides proactive mentorship and guidance to professors and Tunxis personnel.

Plan and teach transition curriculum.

Assist students with completion of resumes and portfolios.

Assist with securing accommodations through the Academic Success Center.

Assist students with class selections with Academic Advisor.

Assist in teaching the executive function skills needed for success at a collegiate level.

Assist with securing student internships and job development placements for students in the community.

Monitors student progress using Therap.

Liaison with all professors and staff when necessary.

Attends and participates in academic meetings as needed.

Provide technical assistance when needed.

On-going completion of outcome-based data in accordance with transition curriculum, Favarh policy and procedures, and educational IEP goals.

Participates and supports students at academic meetings as needed.

Assist in travel training for students.

Communicate information between management, staff of Host Site, staff of business/community, and agency regarding program operations.

Qualifications:

Experience:

A minimum of one year of experience working in the field of Health, Education, Rehabilitation and/or Residential Services is preferred.

Education:

High School Diploma or GED is required.

Must be able to assist students with academics at a collegiate level.

Physical Restraint training/certification is required within 6 months of hire to retain position.

Must possess a valid and in good standing State of Connecticut Driver's License as required

Other:

Ability to proficiently navigate through organizational web-based client documentation software system-Therap.

Serves as a model Favarh employee while performing duties and responsibilities and in the presence of students, their families, host site and the Favarh community.

Employee appearance and behavior should only be perceived by others as appropriate, professional, and serve as an example for others to follow.

Ability to work a flexible work schedule to meet the requirements of the Transition Department.
