Job Responsibilities * Conduct comprehensive assessments of residents to determine their healthcare needs and preferences.
* Collaborate with nursing staff and interdisciplinary teams to develop individualized care plans.
* Monitor and evaluate the effectiveness of care plans, making adjustments as necessary.
* Provide training and support to staff on assessment tools and documentation requirements.
* Ensure compliance with state and federal regulations regarding resident assessments and care planning.
Minimum Qualifications * Current Registered Nurse (RN) license in the state of practice.
* Bachelor's degree in Nursing or a related field.
* Minimum of 2 years of clinical experience in a healthcare setting.
Preferred Qualifications * Experience in long-term care or geriatric nursing.
* Previous experience in a coordinator or supervisory role.
* Certification in gerontological nursing (CGN) or related specialty.
