Baptist AgeWell is seeking a compassionate full-time Nurse PractitionerHomecare to join our team.

This is a primary care role where you will visit patients at home or in other settings.

You must be comfortable and have experience working with adults with chronic medical conditions in a homecare setting.

Good bedside manner is key for success in this role.

You must possess the interpersonal skills to work with patients and families during a pivotal time.

What You Can Expect • This Nurse Practitioner will treat patients in their residence, home, ILF, or ALF.

• We provide Epic EHR System.

• A collaborative team with 4 other Advanced Practice Providers with Medical Assistants and support staff.

• Established geriatric patient panel with some palliative care patients.

• You will treat 6 to 7 patients a day.

• Hours are Monday through Friday from 8 AM to 5 PM.

• Call schedule is 2 times per year.

• The patient panel is aslo fed from Baptist Medical Center Nassau All About the Benefits • Compensation is $114K to $140K+ based on experience with the ability to increase your compensation based on our productivity model.

What You Will Need • Master’s degree - Nursing, required • 1 year of work experience • Certified and Licensed APRN Education Required • Master's Degree – Nursing Education Preferred • Master's Degree – Nursing Experience • At least 1 year APRN Experience Required.

License and Certification • DEA Controlled Substances Registration Required • Certified Nurse Practitioner Required • Family Nurse Practitioner (FNP) Preferred • Adult-Gerontology Acute Care Nurse Practitioner (AGACNP) Preferred • Adult-Gerontology Nurse Practitioner (AGNP) Preferred • Psychiatric-Mental health Nurse Practitioner (PMHNP) Preferred • Licensed Advanced Registered Nurse Practitioner Required • Autonomous Advanced Practice Registered Nurse Preferred
