What You’ll Do

Perform assessments and create personalized care plans

Educate patients and families about medications and treatment

Coordinate care with providers and home health teams

Accurately document patient progress using OASIS

What You’ll Need

Active Arizona RN license

Home health experience, including OASIS documentation

Current CPR/BLS certification (or willingness to obtain)

Valid driver’s license and reliable transportation

TB test & fingerprint clearance (we can assist)

Bilingual in Spanish? That’s a plus!

Graduate of an accredited RN program
