Complex care, Incoming patients are invite-only

Seeing patients 16 and up, 8-10 patients per day

The phone call is shared 1 in 5

onsite infusion center

BLS certification and a strong focus on customer service is required

Experience in population health management and working with psychosocially and medically complex patients is strongly preferred

collaborate with a team that includes a nurse practitioner, medical assistant, nurse care manager, clinical social worker, and office coordinator
