RN Case Manager/Utilization Review

*Experience* - Two years case/resource management and utilization management is required

*Job Duties:*

* Coordinate patient care plans across the continuum, from admission to discharge, ensuring cost-effective and quality outcomes.

* Perform comprehensive assessments to determine patients’ needs, eligibility for services, and appropriate care levels.

* Collaborate with physicians, nurses, social workers, and other healthcare providers to develop individualized treatment and discharge plans.

* Monitor and evaluate the effectiveness of care plans, making adjustments as needed based on patient progress.

Pay: $3,200.00 - $3,400.00 per week
