EY RESPONSIBILITIES / ESSENTIAL FUNCTIONS
Has working knowledge of all State and Federal laws and regulations affecting the practice of pharmacy.
Assumes responsibilities of the Clinical Pharmacy Manager, as assigned, in his/her absence.
Develop and maintain an area of clinical expertise in critical care providing clinical pharmacy services for patients and health care professionals in this practice area. Must also maintain clinical skills knowledge to cross cover for Infectious Diseases Clinician in their absence.
Oversees all aspects of maintaining and supporting clinical services/programs, including staff development and competency assessment for the following: Antimicrobial Stewardship, Pharmacokinetics, Continuous Renal Replacement Therapy (CRRT), Nutrition Support, Anticoagulation Services, Automatic Renal Dosing, IV to PO Conversion Program and pharmacist's order entry assessment.
Participates as needed on the following hospital committees: ICU, Infection Control, Nutrition, P&T Committees and Core Measure Teams.
Monitor patient drug therapy for patients in the clinical practice area to ensure efficacious, safe, and cost effective drug therapy.
Provides drug information to physicians, staff, and co-workers through direct contact, committee participation and quarterly staff educational programs.
Promotes rational and economic drug use through professional interactions with the medical staff and medical staff committees.
Monitors the medication use process on a routine basis and reports inconsistencies to improve mediation safety within the hospital.
Maintains a clinical pharmacy rotation for students/residents. Participates in activities of the Residency Advisory Group. Supports and mentors resident research and other scholarly activities.
Documents improvements in patient outcomes and cost containment efforts in the department's clinical intervention system. Provides the Clinical Pharmacy Manager with a quarterly performance improvement report / graphs that depicts the Clinician's patient care activities. Participates in other performance improvement activities, as assigned by the Clinical Pharmacy Manager.
Assists with performance evaluations and annual competencies as directed by the Clinical Pharmacy Manager.
Maintains policies and procedures related to clinical pharmacy services.
Maintains workload statistics and other productivity parameters to promote the success of the clinical pharmacy services.
Required to staff one day per week in a patient care area providing order entry and clinical pharmacy services. Also required to staff the Nutrition Support Service one weekend per team rotation.
Assist with the training of new employees, pharmacokinetics training and staff education (Clinical Updates).
Assists in the development of, and supports compliance with, a departmental budget.
Provides immediate, direct supervision by process defined by leadership to unlicensed pharmacy students and interns.
Qualifications
Must hold a current license to practice pharmacy in the State of Alabama. Must be an Alabama pharmacy preceptor or eligible. The candidate must have developed an area of personal clinical practice, contributing regularly to patient care, and has exhibited a commitment to maintain a level of practice well beyond that required of staff practitioners.
This position requires a:
B.S. or Pharm.D. Degree required, with a
Completion of an ASHP Accredited PGY1 or Critical Care Specialty Residency or a minimum of four years post graduate experience in direct patient care in a critical care environment
ACCP board certification (BCPS) is preferred. ACLS Certification is preferred. Experience precepting and training pharmacy students and residents are preferred. Membership and active participation in professional pharmacy organizations is expected. Leadership experience is preferred. Work experience on multidisciplinary teams and participating on Medical Staff Committees is preferred. Must possess effective communication and supervisory skills. Must be able to read, write legibly, speak and comprehend English.
