Are you ready to make a real difference in patient care? Join Gallagher Hospice as a Licensed Practical Nurse (LPN/LVN) and provide compassionate care to those in need!

Key Responsibilities:

Provide direct patient care, including specialized end-of-life care.

Implement and follow comprehensive care plans, including accurate and timely documentation.

Educate patients and families on self-care and treatment plans.

Coordinate hospice services with an interdisciplinary team to ensure quality patient outcomes.

Manage pain and symptoms, ensuring comfort and dignity for patients.

Participate in professional growth and quality improvement activities.

Requirements:

Graduate of an accredited nursing program.

Minimum 1 year of nursing experience (community health/hospice or med/surg preferred).

Active LPN/LVN license in the state.

CPR certified and up-to-date on licensure.

Strong communication and organizational skills.

Valid drivers license with a reliable and insured vehicle.

Passion for working with terminally ill patients and understanding of hospice philosophy.
