Responsibilities:
Conduct comprehensive cardiovascular assessments and diagnostic evaluations for patients within our internal referral network of over 80 Primary Care Providers.
In-house diagnostics include echocardiograms, nuclear, and vascular services.
Collaborate closely with interdisciplinary teams including primary care physicians, advanced practice providers, nurses, and other specialists to ensure comprehensive patient care.
Educate patients and their families about cardiovascular health, disease prevention, and management strategies.
Monitor and adjust treatment plans in response to patient progress, changes in health status, and evolving clinical findings.
Hospital rounding optional.
