Compensation Range: $25-$29/hourly commensurate with experience
Summary:
The Supervisor is responsible for general oversight of service delivery to clients of the CCR program within the Retention & Adherence Department across three clinic sites (East New York Health Center, Downtown Brooklyn Health Center, and Westside Health Center). The Supervisor will provide oversight of all staff assigned to CCR, collaborating on service delivery including care plan development, engagement, adherence, linkage to other supportive services, and case finding. Additionally, the Supervisor will determine client eligibility and routinely review staff caseloads to assess client eligibility. The role reports to the Director for Retention and Adherence. This position works collaboratively with a care team that includes the medical provider, client navigators, outreach staff/peers, nurse coordinator, and additional support and administrative staff. Utilizing a multidisciplinary care team approach in a primary care and community setting, team members work with a complex population that includes people who use drugs, people experiencing housing instability/homelessness, people with mental health issues, and others experiencing the effects of social and structural determinants of health. In addition to primary care, this position will collaborate with other Housing Works programs such as Youth and Prevention Services, OASAS Substance Use Treatment, and OMH Mental Health Care.
Essential Duties and Functions:
Responsible for monitoring the service-based aspects of the contract along with delivery of direct services.
Provides direct services which include care coordination, treatment adherence counseling, and crisis intervention.
Provide supervision to the department staff, including Navigators and Outreach Workers
Reviews/approves assessments, reassessments, care plans, PCSMs, self-management assessments, and case conferences.
Monitors programs’ enrollment and discharge trends to ensure retention and quality.
Maintains staff scheduling to ensure adequate program coverage at all times.
Collaborate with key stakeholders at each site, including leadership clinic staff, program staff to ensure effective communication and service delivery
Ensure all client records are maintained in accordance with all state and federal laws, rules and regulations.
Participates in the development and execution of Quality Improvement Initiatives in collaboration with the department and QI Team.
Ensure high-quality client experience by ensuring all client care is coordinated across Housing Works programs and addressing client issues and barriers to care as necessary
Minimum Requirements
College degree required
Knowledge and experience with the dually diagnosed community of HIV/AIDS and mental illness.
Knowledge and experience with state and local policies of NYC affecting persons living with mental illness, HIV/AIDS.
Some Experience in program planning and program operations.
Experience working with homeless populations and active substance users.
