DESCRIPTION:

Hospice Care: provide acute symptom management, goal setting, goal setting, discussion of advance directives and psychosocial support to patients and families facing serious illness and grief

Will work in collaboration with physicians and the Interdisciplinary Team as a Advanced Practice Nurse

Full-time permanent position

NP will be 40 hr/week schedule and participate in the on-call weekend rotation

Coverage provided in the following settings: Nursing Homes, Assisted Living, Hospitals, Homes

REQUIRED:

CO state license

DEA

BLS/CPR

Valid Colorado driver's license and unlimited access to reliable transportation

Adequate automobile insurance

1 year experience

COMPENSATION:
