Job Details

The On Call Center Triage RN is responsible for the emergent triaging of calls received from patients and/or caretakers to for the purpose of telephonically assessing their needs in order to treat emergent situations that have developed during the On Call WEEKEND Center hours

Hospice Triage RN provides patient physical and psychosocial health systems assessments telephonically, supporting the Plan of Care, “Goals of Care” based on the patient’s and caretaker’s needs/wishes, and in coordination with plan developed/placed by the primary RN Case Manager for the patient.

Responsibilities:

Telephonically assess the incoming calls to the On Call Center in order to effectively triage the

emergent patient situations.

Reviews at the beginning of the shift all information via email to the Hospice Call Center Inbox regarding

changes and relevant information received.

Monitors NetSmart Task box hourly in order to dispatch on call visit staff and to ensure timely follow up

and clinical coordination with the On Call Services Field RN.

Places “Tuck-In” call for telephonic follow up and may schedule an RN visit PRN when deemed necessary after triaged.

Escalates appropriate calls/situations to AOC as needed.

Provides telephonic intervention to patients based on the Plan of Care and “Goals of Care”.

Triages call received and includes thorough data collection of the complaint, presenting patient symptoms, and urgency of the situation.

Telephonically assesses the situation for the physical, emotional, and psychosocial needs of the patient and caregivers.

Upon completion of the telephonic assessment, the On Call Services Triage RN will determine the

necessity for a home visit.

Places visit on the On Call Services Field RN’s schedule and notifies the RN telephonically of the priority

of the visit within the existing scheduled patients.

Coordinates and provides pertinent information to the On Call Services Field RN and other IDT members

PRN.

Documents all findings and notifies other team members of changes made via NetSmart and EOS (End of

Shift report) via email.

Assesses the environment for safety within the constraints of telephonic assessment.

Provides and documents teaching provided in NetSmart.

Provides emotional support for both patient and their caretakers, especially during periods of severity of

illness or intensity of services as patient progresses along the hospice continuum.
