Job Details
What You Will Be Doing
Providing exceptional primary care
Conducting office visits for routine and acute issues
Administering annual wellness visits and health risk assessments
Coordinating care with other providers, specialists, and agencies
Leading population health initiatives with the Care Team
Assisting with phone triage and outreach
Educating patients on health conditions, care plans, and treatments
Skills You Need
Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
National certification in a relevant specialty (e.g., Family Nurse Practitioner)
