Job Description
Job Purpose
The Day Program Facilitator provides direct support to clients who participate in the Day Program including developing and facilitating groups, as well as, individual support. The facilitator documents all information required to determine the client's progress towards the achievement of group and individual participant goals.
Essential Functions
Create and facilitate groups that help clients achieve goals and outcomes identified in their Individual Service Plan
Understand and follow client's Individual Service Plan to ensure excellent service provision; provide assistance to program participants as needed
Transport clients as assigned (able to drive a 15-passenger van)
Foster the growth of individuals by teaching and offering opportunities for choice, initiation, and risk-taking
Make preparation and serve lunch for attendees
Provide crisis intervention, conflict resolution, and mediation as needed.
Participate in all staff meetings, trainings and client meetings as assigned.
Must check email and voicemail daily
Utilize Electronic Health Record System to create accurate and timely notes on all services delivered and relevant client information that meets or exceeds agency and state standards
Complete other departmental documentation
Correct all documentation as directed by supervisor
Attend all electronic health record (Foothold) training as required

Educational Requirements
· High School Diploma or GED
Associate degree or further degree in psychology, mental health, or a related field preferred
Work Experience Requirement
· Minimum of two years working with individuals with developmental disabilities, or an equivalent combination of education and experience that provides the knowledge, skills and abilities to perform job duties and responsibilities proficiently.
Position Type/Expected Hours of Work
· Hourly
· Must be available Monday through Friday 8am – 5pm.
· Valid New Jersey Driver’s license is required in good standing.
Special Working Conditions/Physical/Requirements – Working Environment
This job operates in a professional environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
· Must be available for coverage on some weekends and after 5pm when required.
· Must hold a high school diploma.
· Valid New Jersey driver's license is required.
Must hold CPR/First Aid Valid Certification
Successful clearance of NJ background check.
Successful clearance of Drug Screening
Has fulfilled the necessary training requirements and is in good standing in order to bill Medicaid for Support and Community Care Waiver (CCW) Services and CDS
