Are you a compassionate and skilled APRN who thrives on providing comfort, guidance, and expert care during life's most vulnerable moments? We're seeking an experienced Advanced Practice Registered Nurse (APRN) to join our dynamic Hospice team on a PRN basis.
In this essential role, you will deliver patient-centered, holistic care and support-aligned with our mission to provide dignity, peace, and respect throughout the end-of-life journey.
What You'll Do:
Deliver expert nursing care to hospice patients in their home environment.
Assess and manage patient conditions in collaboration with the Hospice Medical Director and Interdisciplinary Team.
Conduct face-to-face recertification visits and regular evaluations to ensure patients remain eligible and receive optimal care.
Maintain thorough and timely documentation to support team communication and care continuity Requirements
What You Bring:
Master's Degree in Nursing from an accredited program.
Current APRN licensure with AANP or ANCC board certification.
Valid driver's license, reliable transportation, and automobile insurance.
Hospice or palliative care experience preferred.
