Summary/Objective

The Licensed Practical Nurse provides nursing services to individuals. The LPN reports to a Registered Nurse and is responsible for working within the scope of their license to ensure that the physical health needs of the clients are met and maintained on an individualized basis in accordance with policies and procedures and current federal, state, and local standards, guidelines, and regulations. LPN will coordinate health needs with members of the individual's team as well as provide necessary training to team members under the direction of a licensed physician, licensed dentist or at the direction of a registered nurse. Under the direction of the RN, the LPN may also be responsible for delegating nursing tasks to certified direct service providers.

Essential Functions

Acting on direction of a licensed physician, licensed dentist or at the direction of a registered nurse administers controlled and non-controlled medication (i.e., oral, rectal, and injection); maintains medication accountability records; maintains medication equipment and supplies; performs skilled nursing procedures (e.g., applies and changes dressings, catharizes); assists nurse in medical procedures; requests, secures and follows up on medical information of residents.

Provides general nursing care to individuals; maintains first-aid supplies and provides first-aid treatment as necessary; screens for communicable and nuisance diseases.

Observes and documents conditions and responses to, and progress of, nursing care treatment and medications; reports unusual developments to the RN.

Supervises serving of diets assuring that individuals unable to feed themselves are properly fed and that special eating utensils are available as needed; notes and records special problems related to eating and fluid intake; reports unusual problems to the Dietitian and Physician; provides instruction and training to residents in personal health-related matters (e.g., nutrition, grooming, hygiene, safety)

Prepares and maintains written records and reports as required; ensures compliance with all reporting standards; accepts on-call rotation

Participates as member of the interdisciplinary team working with other professional staff assessing, developing, implementing, evaluating and modifying individual plans

Under the direction of the RN provides consultation services to staff, residents, parents and significant others regarding health and safety related issues; promotes consistency of programming outside of residential environment

Operates vehicles.

Participates in infection control, pharmacy and therapeutics, committee meetings and others as necessary

Performs other related duties as assigned.

Minimum Qualifications, Experience & Education:

Must be at least 18 years of age

Graduate of an approved school of practical nursing

LPN License - State Board of Nursing

One year experience working as an LPN with intellectually or developmentally disabled individuals preferred.

Must be able to communicate in English, including both spoken and in writing.

Licensure/Certification:

First Aid, CPR - adult, child and infant; delegated nursing

Valid Ohio Driver’s License with Ohio Bureau of Motor Vehicles

Proof of auto insurance.

Special Skills and Requirements:

Must be detail oriented and have competent literacy and writing, documentation, communications and interpersonal abilities.

Must be able to use tools and equipment as required – including pushing/pulling wheelchairs, lifting consumers with Hoyer lifts

Must be able to travel in and out of apartments in order to perform medication passing.

This schedule has a flexible start time between 7:30 a.m. and 9:30 a.m. for a structured 8-hour workday with a dedicated 30-minute lunch break, totaling 8.5 hours on site.
